Neltharus is a dungeon that's best known for its chains, which behavior changes drastically in season 4. Instead of massive damage, they are now a long stun with a damage amp. This should make it more interesting.

Tanks are often surprised by Blazing Slash, probably because it is Magic Damage, and they have not had much time to build their mitigations. Be ready to assist them.
Absolutely interrupt Molten Core from the Qalashi Thaumaturge, unless you want to play hard mode. The other danger in this area comes from them as well, with Magma Conflagration which does very intense damage to a single target. Track it and be ready to use strong ST tools.
Be ready to use your multiple stops on Magma Conflagration as it is the biggest danger here.
Magmatusk is a very tough fight, that scales through the encounter. This makes it an extremely challenging tyrannical boss.
Since the fight becomes exponentially difficult, you should try to add as much damage as you can, especially at the beginning of the fight, put everything you have on the boss. This is honestly the best thing you can do to make your life easier.
Lava Spray creates single target damage, and people (including you, should use a defensive when targeted). This should generally be under control, although it will scale in time. RNG can make it really hurtful if it just keeps targeting the same person.
The issue is of course Magma Eruption. It will start being a moderate burst of damage, but it will last longer and longer with each Magma Tentacle. With time, it will simply feel like a very heavy Rot fight. Make sure to rotate your cooldowns effectively, there are no easy tricks there.
Melee uptime can be tricky on this boss, be very careful of the Lava Wave.
This is the main "chain" zone. They are now a little more "interactive" since they only stun and buff the damage inflicted to the mobs. You might end up wanting to stagger them more now to maximize the usage rather than bursting them all at once.
Use your controls to group the enemies as much as possible for the chains. Beware of the Magma Fist frontals in the chaos.
Expect tons of random damage in this area from Bold Ambush and Bone Toss, which can make it a stressful area to heal. Even more reasons to stagger the chains for better control of the enemies!
Be quick to dispel Imbued Magma and interrupt Burning Roar. Getting silenced can make this whole area instantly more problematic.
If you can block the Qalashi Hunter with LegSweep or RingOfPeace, do not hesitate. Their Bold Ambush are the clear danger here.
Chargath's chains mechanic is very specific, which makes it an annoying boss with pugs.
The main mechanic of the fight is of course the Fiery Focus, which starts with the Grounding Chain on the DPS every 58 sec. Be mindful of it if you use 1 min cooldowns, you should use them early on the first one to account for the slight delay you will accumulate each time.
No more circle around the boss, so your movements will be less constrained during the mechanic, but the burst of damage is still here each time a chain is broken. You depend on your team and their coordination to not get multiple stacks of Slag Eruption, which makes this boss honestly atrocious with pugs, just very difficult with an organized group.
Be sure to be early with your Chiji for the first Fiery Focus combo window. The 58 seconds cycle will put you slightly behind on every iteration, do not let yourself be surprised by this.
This is a rather intensive area for you. Try to interrupt the Qalashi Irontorch when they cast Mote of Combustion as it will create AOE damage when the debuff expires otherwise.
Qalashi Bonesplitter will cast have a dangerous bleed with Dragonbone Axe that you should try to CC, to save you some healing.
Help interrupt and group the Lava Flare. They cannot left alone casting, they would destroy your group.
The Forgestomp from the Qalashi Blacksmith will create unavoidable damage every 18sec that you should be ready for.
Consider this area a warm-up for what is to come.
A very intense healing encounter! Be ready for a fast-paced challenge, with repeating AOE burst, in an encounter that wants you to spread. Technically, you can see this fight as the proof that you made it as healer (until the next level humbles you yet another time).
Forgemaster Gorek is a death machine, on a 31sec clock.
He will jump to his anvil (Might of the Forge), strike his hammer 3 times (Blazing Hammer) and then send his new toys to 3 victims (Blazing Aegis) for a total of 15 seconds of joyous chaos, with 1-shot swirlies everywhere. He will then proceed onto murdering the tank, but I dare say that is mostly their problem, keep an eye on them still. When you think you are about to recover, he is usually mid-air for the next session.
What matters the most on this fight is to not loose your pace. 2 things could disturb you. The swirlies of course, but also LOS problems. Between the stairs around the anvil and the anvil itself, you could loose vision of your teammates. There is no easy solution for that, try to adopt a central position (yes in the middle of swirlies) and make sure to let DPS know that it can be an issue so that they think of their positioning as well.
If you are running out of cooldowns, request defensives or off-heals from your teammates, most people know that this fight is very hard on you, they should be careful.
One last trick that can come really handy. Blazing Aegis targets people in order, and it is possible to Vanish/Invis/Shadowmeld it. If the first target does it, it cancels the entire mechanic. This really will help you, so share the tip!
The Apex Blazewing is dangerous for the tank with Scalding Chomp and to the whole group with Candescent Tempest every 27sec, one of these annoying bursts that also push you away.
The main other mechanic to know about is Molten Barrier on the Qalashi Lavamancer. Be ready for a burst of damage when the shield is broken. Nothing too bad however.
Really, the pressure will be on your tank in this part. Keep an eye on them!
With a slight rework to the encounter, we should expect Warlord Sargha to feel more fair.
This boss is mostly about mechanic, and the damage pattern is not that intense, but the amount of movement and mechanics involved make everything harder.
Mainly, there will almost always be a Molten Gold applied to a member of the group. It lasts for 18 seconds, overlasting almost any defensive, and you will need to keep a constant eye on them, make sure you have a clear way to know who has it on them. Dispel Curse of the Dragon Hoard from them as a priority (if you can).
The main mechanic otherwise is of course Magma Shield and the scaling damage of Flame Eruption that comes with it, every 45 seconds or so. You will rely on your whole team (including you) to use the items asap and break the shield before it scales. Try to ramp up some HOTs or combos before it starts as it will be hard to stand more than a second once the rain of fire starts.
